There is also a consent form, BOTH need to be signed.

Nebraska Statute requires a physical examination by a NEBRASKA physician if you are:

    • An incoming Freshman (gr.9).

    • An incoming 7th grader with an updated immunization report.

    • A Transfer students.

    • An incoming kindergartener with an updated immunization report.

    • Participating in any Sport, PE, and/or Weights class.

Below is a letter the School Board Policy Committee and Mr. Dahlberg crafted for your use, to comment on the proposed health standards that the Nebraska Department of Education will be discussing on Friday, April 2nd. We urge parents and guardians to contact Robin Stevens and the NDE Standards Committee by either email or mail or both. We feel these standards are an overreach of NDE and the content is not appropriate for teachers to have to teach.

NOTICE:
Thedford Public Schools is doing some cleaning of files. We will be destroying old SPED files that are more than 5 years old from the graduation date of the student. If you would like these files for your records please contact the school before September 4th as we will shred them September 5th. 

009.03M Retention and Destruction of Information and Records 

  • 009.03M1 The school district or approved cooperative shall retain special education records for five (5) years after the completion of the activities for which special education funds were used. 

  • 009.03M2 The school district or approved cooperative shall inform parents when personally identifiable information collected, maintained, or used is no longer needed to provide educational services to the child. 

  • 009.03M3 The information which is no longer necessary to provide educational services to the child, must be destroyed at the request of the parents. However, a permanent record of a student's name, address, and phone number, his or her grades, attendance record, classes attended, grade level completed, and year completed may be maintained without time limitation.
